Job Description

Location:
Chino, CA Schedule:
Monday, Wednesday, Friday, and Sunday 10:00 AM - 6:00 PM Position Overview We are seeking a compassionate and experienced Licensed Vocational Nurse (LVN) to provide one-on-one private duty nursing care for a 76-year-old client with advanced dementia. The ideal candidate will be skilled in medication administration, patient monitoring, and providing high-quality care in a home setting.
Patient Profile:
76 years old Approximately 60 lbs. Advanced dementia Total care patient Wheelchair dependent Requires assistance with all activities of daily living
Responsibilities:
Administer 5-8 medications per shift as prescribed Provide skilled nursing assessments and monitoring Assist with feeding three times daily Perform personal care and shower assistance three times per week Monitor for changes in condition and report findings appropriately Maintain accurate nursing documentation Collaborate with family members and care team Ensure patient safety, comfort, and dignity
Requirements:
Current California LVN license in good standing Current CPR/BLS certification Experience with dementia patients preferred Strong assessment and documentation skills Reliable transportation and punctual attendance Why Join Our Team?
