ASAP Pay Package:
Pay Rate:
$16/hr on W2 + $700 Weekly Stipend for meals and lodgings.
Job Overview:
We are seeking a Lead Care Management Coordinator (LVN) to manage clinical intake processes, coordinate referrals, and ensure smooth communication between patients, families, and healthcare teams. This role involves supervising workflow, verifying insurance, and supporting home health scheduling and coordination.
Requirements:
Active LVN (Licensed Vocational Nurse) Certification
  • Required Minimum 1 Year of Experience
  • Required Clinical Intake Coordinator Experience
  • Required Strong communication and coordination skills Experience in home health processing & referral scheduling

Day Shift:
Monday
  • Friday 8:00 AM
  • 5:00 PM (5x8s) 40 Hours per
