Implements current nursing practice following a comprehensive assessment and the plan of care. Provide accurate and timely documentation of patient services to reflect the plan of care. Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care. Participates in coordination of palliative care services, appropriately reporting the identified needs to the interdisciplinary group. Uses equipment and supplies effectively and efficiently. Provides appropriate pain/symptom management. Evaluates patient's response to treatments/medications. Participates in personal, professional growth and development. Also participates in quality assessment performance improvement teams and activities. Performs other duties as assigned by the RN or Lead NP Education and Experience Currently licensed as Vocational Nurse in the state of California. One (1) to three (3) years of nursing experience. Community health/hospice or medical/surgical and palliative care experience is preferred. Possesses and maintains current CPR certification as required by organizational policy. Self-starter that can work independently and collaboratively, prioritize tasks and has initiative and excitement to take on unfamiliar tasks. Advanced knowledge of word processing, graphic presentation and computer software related to specific tasks Demonstrated excellent computer and word processing skills with special emphasis on calendaring, presentation, and spreadsheet capabilities Working knowledge of company policies, procedures, and operations Excellent composition, grammar, and business language skills Excellent communication and interpersonal skills with the ability to effectively communicate with all levels of management, patients, and family members.
