South Los Angeles, CA What You'll Do:
The Impact You'll Make As an LVN, you will provide nursing support, care coordination, medication management, and client-centered services to individuals with co-occurring medical, mental health, and substance use conditions. You will work closely with an interdisciplinary team to help clients access and maintain critical healthcare services while supporting overall wellness and stability. Medical Care & Coordination Provide basic nursing services, including injections, medication administration, and monitoring. Coordinate and schedule primary care and specialty medical appointments for clients. Maintain medication logs in collaboration with the Program Manager for clients receiving psychiatry services through community partners. Coordinate with pharmacies to ensure timely medication delivery and continuity of care. Provide patient education and supportive services to promote medication compliance and wellness. Collaboration & Integrated Care Work collaboratively with an interdisciplinary team to support clients with complex medical, mental health, and substance use needs. Communicate effectively with internal staff, external providers, pharmacies, and community partners to ensure continuity of care. Provide crisis intervention support and coordinate emergency response services when necessary. Participate in weekly staff meetings, case conferences, and supervision sessions. Documentation & Compliance Maintain accurate and timely documentation within the designated electronic health record or charting system. Support clients in navigating Medi-Cal benefits and healthcare systems. Maintain compliance with HIPAA, confidentiality standards, and agency policies and procedures. Operational Support Assist with day-to-day program operations and additional duties assigned by leadership to support quality client care and program effectiveness.
What You'll Bring:
Your Qualifications Education and Experience Current Licensed Vocational Nurse (LVN) license in the State of California required. One to three (1-3) years of clinical experience working with individuals with serious mental illness (SMI), homelessness, and/or complex populations preferred. Experience in behavioral health, social services, supportive housing, or community-based care settings strongly preferred. Current CPR Certification preferred. Skills and Abilities Strong understanding of confidentiality standards and HIPAA requirements. Ability to perform effectively in high-stress situations and work with clients experiencing behavioral health crises or emotional distress. Strong organizational, communication, and care coordination skills. Familiarity with Electronic Health Records (EHR) and related healthcare technology systems. Ability to work collaboratively within a multidisciplinary team environment. Requirements Valid California Driver License, clean driving record, and insurability under Alcott's insurance policy required. Live Scan fingerprint clearance required. Why Join Alcott?
