Position Summary:

We are seeking dedicated and experienced Licensed Vocational Nurses (LVNs) to provide private duty, one-on-one care to individuals in their homes in Santa Barbara, CA 93110. Key Responsibilities:

Deliver skilled nursing care to clients in accordance with physician orders and individualized care plans. Monitor patient conditions and report changes to the RN Case Manager and physician as needed. Administer medications and treatments as prescribed within scope of practice. Provide assistance with ADLs (activities of daily living), including hygiene, mobility, feeding, and toileting. Ensure proper documentation and timely charting of assessments, treatments, and interventions. Educate the client and family on disease management, medication administration, and safety measures. Maintain a safe, clean, and supportive home environment for the client. Collaborate with the multidisciplinary care team to optimize outcomes and enhance the patient experience.

Qualifications:

Current and valid LVN license in the state of California. Minimum 3 years of LVN experience. Minimum 1 year of home health, private duty, or acute care. Current CPR certification (American Heart Association or equivalent). Strong clinical and critical thinking skills. Excellent interpersonal and communication skills. Ability to work independently with minimal supervision. Reliable transportation and valid driver's license (if travel required). Experience with tracheostomy, ventilators, or G-tube care is a plus (training available if needed).

Shift Details:

SET 12 HOUR SHIFT SCHEDULE

SUNDAY-WEDNESDAY 8 PM

TO 8 AM AND/OR THURSDAY-SATURDAY 8 PM TO 8 AM
