A successful Licensed Vocational Nurse (LVN) must be passionate about healthcare and want to make a difference in the lives of others while acting as a mission-driven catalyst to help Livingston Community Health deliver the best quality of care and excellent service to our patients and their families. The principal function of the Licensed Vocational Nurse is to provide safe patient care by assisting Clinic Providers with treatments, procedures, and administrative duties as defined in the Licensed Vocational Nurse Scope of Practice. The Licensed Vocational Nurse will demonstrate competence in patient care for the infant, child, adolescent, and adult patient populations. The LVN will ensure the delivery of quality patient care and efficiency of patient flow. Essential Functions, Duties, and Responsibilities Core Patient Care Pre-visit planning for preventative care and chronic care. Collect patient history, document symptoms, and patient concerns in the medical record. Reconcile current medications. Take and record vital signs (blood pressure, temperature, pulse, respiration). Prepare patients for exams and procedures. Inform care provider of patient questions, concerns, or changes in health/medication. Assist physicians, nurse practitioners, or physician assistants during exams, medication-assisted treatment, OB/GYN services, and other in-office specialized services (as permitted by state scope). Clinical Tasks Administer medications (oral, topical, and injections as permitted by state scope). Perform basic wound care and dressing changes. Draw blood and collect lab specimens. Perform point-of-care testing (e.g., glucose checks, urinalysis). Administer immunizations. Remove used/dirty instruments to be autoclaved. Respond to all emergency codes. Care Coordination & Patient Support Provide patient education on medications, treatment plans, and follow-up care. Answer patient questions and respond to basic clinical concerns. Patient inquiries must be answered within 48 hours and recorded in the electronic health record (EHR). Coordinate referrals, lab orders, and follow-up appointments. Support chronic disease management (e.g., diabetes, hypertension). Documentation & Compliance Follow all established nursing workflows, clinical protocols, policies, and procedures. Accurately document all care in the EHR. Maintain patient confidentiality in accordance with HIPAA. Follow clinic protocols, infection control standards, and safety procedures. Conduct and record monthly clinic site audits. Administrative Support Schedule patient appointments and manage clinic flow. Request insurance prior authorizations for medication, durable medical equipment, and supplies. Process prescription orders and refills per provider direction. Track lab orders through receipt of results. Maintain and stock exam rooms and medical supplies. Inventory weekly and monthly medication and order medications, vaccinations, and other needed supplies. Team Collaboration Monitor and assist with patient flow. Demonstrate competence with the mission, vision, and values of the organization, providing quality services to the community. Maintain confidentiality and respect for all sensitive information and protect company assets. Contribute and provide support as part of the team by fostering professional staff interactions and maintaining open communication and respect across the organization. Present a positive view of the Company to the community and partners. Attend and actively participate in all meetings (e.g., department meetings, program meetings, staff meetings) and other activities as required or assigned. Willingness to travel to all clinic locations to support quality patient care and operational needs. Comply with all Company policies and procedures, including but not limited to practicing workplace safety and reporting work-related injuries. Perform other work-related duties as assigned. Duties and responsibilities may be added, deleted, or changed at any time at the direction of leadership, formally or informally, either verbally or in writing. Education, Knowledge, Skills, and Abilities Education and Experience Completion of an accredited vocational nursing program. Minimum one year of experience as an LVN in an outpatient clinic. Experience at an FQHC or community health setting preferred. Knowledge, Skills, and Abilities Must hold a current California Vocational Nurse License (LVN) in good standing. Basic Life Support (BLS) Certification from the AHA. Valid Driver's License (only if position required regular driving). Knowledge, Skills, and Abilities Solid knowledge of nursing principles and patient care practices. Medication administration, including dosage, routes, and side effects. Infection control standards and universal precautions. Clinical procedures (vital signs, wound care, specimen collection). Documentation standards and use of electronic health records (EHR). Patient privacy requirements under HIPAA. Scope of practice guidelines under the California Board of Vocational Nursing and Psychiatric Technicians. Provide clear patient education on treatment plans and medications. Manage time and prioritize tasks in a fast-paced clinic environment. Recognize and report abnormal findings to providers. Patient rights and confidentiality requirements. Cultural competency and diversity awareness in healthcare. Effective communication (verbal and written with patients, families, and staff). Customer service / patient service oriented. Computer proficiency (EHR systems, email, scheduling tools). Time management and prioritization. Attention to detail and accuracy. Problem-solving and critical thinking. Team collaboration and interdisciplinary coordination. Documentation and recordkeeping. Conflict resolution and de-escalation. Work effectively in a fast-paced environment. Adapt to changing priorities and workflows. Demonstrate empathy and compassion toward patients. Follow detailed instructions and comply with protocols. Remain calm and professional in stressful situations. Work independently with minimal supervision. Demonstrate reliability and accountability.
