Duties:
- Provide direct patient care, including administering medications, performing treatments, and monitoring vital signs.
- Assist with patient assessments and develop care plans in collaboration with the healthcare team.
- Maintain accurate and up-to-date medical records and documentation.
- Utilize electronic medical record systems to input and retrieve patient information.
- Follow aseptic techniques to prevent infection and promote patient safety.
- Collaborate with interdisciplinary teams to coordinate patient care and ensure optimal outcomes.
- Provide education and support to patients and their families regarding their healthcare needs.
- Assist with procedures such as wound care, suture removal, and specimen collection.
Qualifications:
- Valid Licensed Vocational Nurse (LVN) license in the state of California.
- Experience working in a healthcare setting, preferably in primary care or memory care.
- Proficient in medical record systems and electronic health records (EHR).
- Knowledge of aseptic technique and infection control practices.
- Ability to perform patient assessments and recognize changes in condition.
- Familiarity with neonatal ventilators is a plus.
- Experience working with individuals with developmental disabilities is desirable.
- Strong communication skills and the ability to work effectively as part of a team.
- Ability to administer injections and other medications as required.
