Job Description
Overview We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) to join our healthcare team. The ideal candidate will possess strong clinical skills, experience with diverse patient populations, and proficiency in various EMR/EHR charting systems such as OASIS. This role involves providing high-quality patient care across multiple settings, including long-term care, home health care, and community health environments. The LVN will be responsible for performing comprehensive assessments, supporting patient recovery and well-being through expert nursing practices. Experience in home health care is preferred. Responsibilities Conduct thorough patient assessments including vital signs, physical examinations, and physiological evaluations. Document all patient interactions accurately using EMR/EHR charting systems such as OASIS. Collaborate with interdisciplinary teams to develop and implement individualized care plans for patients. Provide education on medication administration, disease management (e.g., diabetes or hypertension), nutrition, and health coaching to patients and their families. Support infection control practices and ensure compliance with HIPAA regulations to protect patient confidentiality. Respond effectively to urgent situations. Requirements Valid LVN license in the state of practice with current certification. Proven experience in diverse clinical settings such as home health care, hospice care, and nursing homes. Demonstrated proficiency with EMR/EHR charting systems like OASIS. Knowledge of medical terminology. Strong clinical skills in areas such as wound care and specimen collection. Ability to work independently while collaborating effectively within multidisciplinary teams. Excellent communication skills with the ability to educate patients on treatment plans and health management strategies.
