Overview Join our dynamic healthcare team as a Licensed Vocational Nurse (LVN) - For Home Health Visits, where your expertise will make a meaningful difference in patient care across diverse settings. Responsibilities Conduct thorough patient assessments including vital signs, physical examinations, and detailed health histories to inform personalized care plans. Administer medications via injections, IV infusions, and other methods while adhering strictly to HIPAA regulations and safety protocols. Manage complex medical procedures such as catheterization, tube feeding, wound suturing, and sterile dressing changes with precision and aseptic technique. Utilize electronic health record (EHR) systems like Epic or Cerner to document patient interactions accurately and efficiently. Provide specialized care for individuals with developmental disabilities, dementia (including Alzheimer's and memory care), and those requiring hospice or palliative support. Support acute pain management strategies and assist with emergency interventions including airway management and ventilator support when necessary. Collaborate with multidisciplinary teams on discharge planning, case management, and coordination of post-acute or home & community care services. Perform specimen collection/processing, diagnostic evaluations, and participate in infection control practices to maintain the highest standards of safety. Educate patients and families on health coaching topics such as medication administration, nutrition, disease management (including infectious diseases), and lifestyle modifications. Experience Valid state licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN). Home Health Experience Proven experience in diverse clinical environments such as hospitals (including ICU or Level I Trauma Centers), nursing homes, outpatient clinics, or home care settings. Critical care experience highly valued; familiarity with ICU protocols, emergency medicine procedures, or Level II trauma center operations is preferred. Hands-on experience with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks for accurate documentation. Knowledge of ICD coding (ICD-9/ICD-10), CPT coding for procedures, and medical terminology essential for precise recordkeeping. Demonstrated ability working with special populations such as pediatrics (including neonatal ventilator care), geriatrics (dementia/dementia care), or individuals with disabilities. Strong physiology knowledge combined with patient assessment skills to evaluate complex cases involving acute pain management or infectious disease care. Additional experience in IV insertion/infusion therapy, phlebotomy, sterile processing techniques, or surgical procedures like suturing enhances your candidacy.
