Active LVN/LPN license in the practicing state Graduate of an accredited LVN/LPN program BLS certification Skilled Nursing or Long-Term Care experience (preferred) Knowledge of state nursing regulations and SNF protocols Ability to lift, reposition, and assist residents as needed

Responsibilities:

Provide direct patient care according to care plans and physician orders Administer medications and treatments as prescribed Monitor and document patient conditions, vital signs, and changes in status Assist with admissions, discharges, and transfers Perform wound care and assist with rehabilitative services Collaborate with RNs, physicians, and interdisciplinary care teams Educate residents and families on care plans and treatments Maintain accurate and timely documentation Follow facility policies, state regulations, and infection control standards
