Job Description

Job Summary We are seeking a dedicated and energetic Licensed Vocational Nurse (LVN) to join our dynamic healthcare team. In this vital role, you will provide compassionate patient care across a variety of settings, including long-term care facilities, assisted living communities, hospitals, and outpatient clinics. Your expertise will ensure that patients receive high-quality nursing services, from routine assessments to complex medical procedures. This position offers an exciting opportunity to make a meaningful difference in patients' lives while working in a fast-paced and supportive environment. Responsibilities Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems such as Epic or Cerner. Administer medications and injections accurately following physician orders while adhering to HIPAA regulations and aseptic techniques. Assist with medical procedures such as IV infusion, catheterization, tube feeding, wound suturing, and sterile processing. Monitor patients' conditions closely, including airway management, ventilator support, pain management, and acute care interventions in settings like ICU or emergency rooms. Collaborate with interdisciplinary teams on discharge planning, case management, and coordination of post-acute or home & community care services. Conduct patient education on health coaching topics such as medication administration, nutrition, dementia care, Alzheimer's care, and chronic disease management. Maintain accurate medical documentation of patient care activities, specimen collection/processing, CPT coding for procedures like botulinum toxin treatments or spinal taps. Ensure infection control protocols are followed rigorously to prevent infectious disease transmission in all clinical environments. Support triage processes by assessing urgent patient needs in outpatient or urgent care settings and prioritizing interventions accordingly. Experience Valid LVN license with current state certification required; additional certifications such as Basic Life Support (BLS) preferred. Proven experience working with diverse patient populations including geriatrics, pediatrics, neonatal ventilator care, or individuals with developmental disabilities. Prior clinical experience in hospital medicine, medical-surgical units, ICU (Level I & II Trauma Center), emergency room or trauma center environments highly desirable. Hands-on experience with EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks for accurate medical documentation and patient records management. Demonstrated proficiency in performing venipuncture (venipuncture), phlebotomy, specimen collection/processing and medication administration including injections and IV insertion. Knowledge of ICD coding (ICD-9/ICD-10), CPT coding for procedures performed in outpatient or surgical settings is advantageous. Experience working with individuals with disabilities or those requiring specialized dementia or Alzheimer's care is a plus.
