School-Based Licensed Practical Nurse (LPN) | Rohnert Park, California ProCare Therapy - 3.9 Rohnert Park, CA Job Details Contract $21.81 - $45.00 an hour 14 hours ago Benefits Health insurance Dental insurance Flexible spending account Vision insurance 401(k) matching Qualifications Diabetes care Full Job Description Active Licensed Practical Nurse (LPN) license and minimum 1+ years Licensed Practical Nurse (LPN) experience required. Applicants who do not meet these qualifications will not be considered. Play a vital role in supporting student health as a Health Aide/Diabetic Care Support professional near Rohnert Park, CA. In this rewarding contract position, you'll work one-on-one with a student attending a non-public school in Santa Rosa, providing essential diabetes care during the school day. Your attention and expertise will ensure the student's safety and well-being from the Extended School Year (ESY) beginning June 15, continuing through the 2026-2027 academic year.
Key Responsibilities:
Monitor glucose levels using a Continuous Glucose Monitor (CGM). Administer insulin at lunch according to prescribed instructions. Accompany and support the student during transitions between school locations, if needed. Communicate updates and health needs with school staff and caregivers. Respond promptly and effectively to diabetes-related concerns, maintaining a safe environment.
Qualifications:
Licensed Nurse (RN or LVN/LPN): Valid RN or LVN/LPN license. Proficient in insulin administration via syringe. Available for approximately 2 hours each day to monitor and support the student pre- and post-lunch.
Health Aide/Diabetic Care Assistant:
Previous experience assisting individuals with diabetes. Confidence in glucose monitoring and diabetes management support. Willingness to administer insulin per school district policy after appropriate training. Dependable transportation for accompanying student between school sites, as applicable.
Schedule Highlights:
Begins June 15 for ESY, extends through the academic year. Involves morning support at one school and potential afternoon support at a second school in Santa Rosa. Downtime is common during the day while on site and available for support.
