Qualifications:

1. Holds a current license from the California Board of Vocational Nursing. 2. Is 18 years of age or older. B. 3. The LVN will provide the center with a health examination, signed by a physician, and a TB clearance, which is not more than 12 months prior to employment or within seven days of employment and shall provide the center a current certificate of CPR and First Aid training. Duties The duties of the LVN include: Providing observation and monitoring of the participant's health status. 1. Documenting all services provided on the same day the services were provided. 2. Providing nursing care within scope of practice for participants as specified in the individual plan of care. 3. Providing health education and counseling to participants and their families, when indicated, in their plans of care, within his or her scope of practice. 4. Providing personal care services for the participants as indicated on the individual plan of care. 5. Providing continence training program as indicated in the plans of care. 6. Providing emergency care, including arrangements for or directly providing transportation when necessary. 7. Documentation of daily participant monitoring, and any unusual findings. 8. The LVN reports directly to the RN. 9. Will be knowledgeable of

ADHC/CBAS

laws, regulations, CBAS certification standards, standards of participation and policies and procedures. This will be ensured through ongoing staff training and education.
