Current LVN licensure by the California Board of Registered Nursing MDS certification preferred Degree from accredited school of nursing Current CPR certification Two years in long-term care experience preferred Minimum of 1 year of experience completing and submittingMDS Job Summary:
Manages and coordinates resident care from admission through discharge within SNF and occasionally, residential living; collaborates with other interdisciplinary health care team members including resident and responsible party; delegates responsibilities to nursing staff. Assesses residents identifies problems, plans, implements, and evaluates nursing interventions, reviewing and revising resident care plans as needed. Provides direct therapeutic, restorative, and preventive care to residents with identified health needs. Provides direction, supervision, and evaluation of unlicensed and licensed personnel in accordance with the policies and procedures of the facility.Essential Job Duties:
As assigned by the MDS Coordinator, contributes to initial care plans and monitors care plans on an ongoing basis for accuracy, completeness and timeliness. Assists MDS Coordinator with monitoring the output of staff nurses responsible for completing assessments (admission, quarterly, change of condition and annual). Completes resident assessment as needed. Supports follow-up of goals and plans developed at interdisciplinary care plan meetings. Assists the facility in following state and federal regulations in regard to resident care and documentation. All Employees must be able to pass background checks (fitness for duty physical, fingerprinting, employment references) as required by a licensed residential care facility.
