The LVN collaborates with residents, families, physicians, pharmacies, and the interdisciplinary team to promote quality resident care and maintain regulatory compliance. Essential Duties and ResponsibilitiesResident Care Assess and monitor residents for changes in physical, mental, and emotional condition. Perform nursing assessments within the LVN scope of practice. Report changes in condition promptly to the physician, Administrator, and responsible party. Coordinate physician appointments and follow-up care. Assist with admissions, transfers, and discharges. Participate in resident care conferences. Update resident service plans and health records as appropriate. Medication Management Oversee medication management in accordance with physician orders and RCFE regulations. Review Medication Administration Records (MARs) for accuracy and completeness. Monitor residents for medication effectiveness and adverse reactions. Coordinate medication refills with pharmacies. Ensure proper storage, labeling, and disposal of medications. Investigate medication variances and implement corrective actions. Clinical Oversight Supervise and support Med-Techs and caregivers regarding resident health concerns. Provide guidance on infection prevention and control practices. Assist with wound observation and coordination of treatment as ordered by the physician. Monitor residents receiving hospice or home health services. Coordinate with outside healthcare providers. Documentation Maintain accurate and timely resident documentation. Complete nursing notes and incident follow-up documentation. Ensure resident records remain current and compliant. Maintain confidentiality of resident health information. Regulatory Compliance Ensure compliance with California Title 22 RCFE regulations. Participate in licensing inspections and surveys. Assist with quality assurance and clinical audits. Monitor compliance with resident rights and facility policies. Education and Training Conduct orientation and ongoing in-service education for caregivers and Med-Techs.

Infection control Change in condition reporting Medication observation and reporting Fall prevention Emergency procedures Resident rights Documentation standards Maintain staff competency records as assigned. Communication Communicate effectively with physicians, families, pharmacies, hospice agencies, home health agencies, and laboratory providers. Participate in interdisciplinary meetings. Promote a positive and collaborative work environment. Qualifications Current California Licensed Vocational Nurse (LVN) license in good standing. Current CPR certification. Minimum one year of nursing experience preferred. Experience in assisted living, RCFE, long-term care, or geriatrics preferred. Knowledge of California Title 22 regulations. Strong communication, leadership, and organizational skills. Ability to maintain resident confidentiality. Physical Requirements Ability to stand, walk, bend, lift, and assist residents as necessary. Ability to lift up to 50 pounds with or without reasonable accommodation. Ability to respond promptly to resident emergencies.
