Full Job Description Duties:

• Provide direct patient care, including administering medications, monitoring vital signs, and assisting with procedures.
• Collaborate with physicians and other healthcare professionals to develop and implement patient care plans.
• Document patient information, including medical history, symptoms, and treatment plans.
• Educate patients and their families on healthcare procedures, medications, and self-care techniques.
• Ensure compliance with infection control protocols and maintain a clean and safe environment for patients.
• Perform basic medical procedures such as wound care, disease process teaching, and medication administration.
• Monitor and report any changes in patient condition to the healthcare team.
• Assist in the coordination of patient follow-up care.

Qualifications:

• Valid Licensed Vocational Nurse (LVN) license in the state of practice.
• Strong knowledge of anatomy, physiology, and medical terminology.
• Experience working with electronic health record systems such as Kinnser.
• Proficiency in airway management techniques and emergency response protocols.
• Familiarity with home health skilled nurse care principles and practices.
• Knowledge of infection control procedures to prevent the spread of diseases.
• Ability to maintain accurate medical records and documentation.
• Excellent communication skills to interact effectively with patients, families, and healthcare team members.
