Position We are currently seeking to fill the Licensed Vocational Nurse (LVN) position in San Luis Obispo surrounding areas:
Compensation:
Hourly and Paid Per Visit (PPV) hybrid model (negotiable based on experience, skill level, availability, etc.) Opportunity to accumulate overtime This position is responsible for providing direct patient care under the supervision of a Registered Nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with team members to help achieve positive patient outcomes. The LVN will work with colleagues under the guidance of the DPCS. Job Description and Responsibilities Provides direct patient care as defined in the State Nurse Practice Act, including specialized duties related to end-of-life care Implements current nursing practice following a comprehensive assessment and plan of care Provides accurate and timely documentation of patient services to reflect the plan of care Assesses and provides patient and family/caregiver education related to diagnosis and plan of care Participates in coordination of hospice services, reporting identified needs to the interdisciplinary group (HHA, OT, PT, MSW, ST, Dietician, Clinical Supervisor) Uses and prepares equipment and supplies effectively while adhering to aseptic technique Provides appropriate pain and symptom management and evaluates patient response to treatments/medications Participates in personal and professional growth, as well as quality improvement activities Performs other duties as assigned by the Registered Nurse The Licensed Vocational Nurse is responsible for providing direct patient care under the supervision of a Registered Nurse. Responsibilities include following the plan of care, providing treatments, and collaborating with the care team to achieve positive patient outcomes. Essential Job Functions / Responsibilities Provides direct patient care as defined in the State Nurse Practice Act Implements the plan of care initiated by the Registered Nurse Provides accurate and timely documentation consistent with the plan of care Assesses and educates patients and caregivers on self-care techniques and diagnosis-related care Participates in coordination of home health services and communicates needs for additional disciplines to the RN/Clinical Supervisor Uses and prepares equipment and supplies efficiently, adhering to aseptic technique Participates in personal and professional growth and development Performs other duties as assigned Experience and Desirable Traits Graduate of an accredited practical or vocational nursing program One year of nursing experience required; three years preferred Community health, hospice, or medical/surgical experience preferred Sensitivity to patient needs with compassion and patience Current California LVN license Strong verbal, written, and organizational skills Licensed driver with insured, reliable vehicle Strong commitment to quality patient care Current CPR certification Understanding of hospice philosophy and end-of-life care Excellent communication and listening skills Positive, "can-do" attitude Strong problem-solving skills Flexibility with schedule, including possible weekends Position Qualifications Graduate of an accredited vocational nursing program Three (3) years of nursing experience preferred Current LVN license in California Compassionate Care was established in 2003 in Fresno County and has grown into one of California's most trusted providers of Home Health, Hospice, and In-Home Care services.
