This role supports residents in a long‑term care environment by delivering safe, effective nursing care in accordance with regulatory standards and physician direction. The LPN/LVN will coordinate care, administer treatments, and ensure resident well‑being while working collaboratively with interdisciplinary healthcare teams.
Qualifications:
Current California LPN/LVN license in good standing Minimum 1 year of LPN/LVN experience Experience in long‑term care or rehabilitation settings preferred Knowledge of patient care standards and safety protocols Ability to recognize and respond to emergency situations Strong attention to detail and documentation skills
Responsibilities:
Perform resident assessments and assist with care planning Administer medications, treatments, and nursing interventions as ordered Monitor resident conditions and report changes to supervising nurse Ensure compliance with all safety protocols and nursing standards Maintain accurate and timely documentation per state and federal regulations Assist in managing medical supplies and inventory Support resident rights and provide patient‑centered care Collaborate with interdisciplinary care teams to ensure continuity of care Maintain a safe, clean, and compliant care environment
