Job Description
Overview Under the supervision of the Medical Clinic Manager, the Licensed Vocational Nurse plans and delivers professional nursing care to patients while maintaining composure and compassion. Responsibilities Performs triage of all patients, ensuring timely appointments as needed. Performs EKG testing, nebulizer treatments, audiograms, visual acuity, and point of care testing. Obtains and prepares laboratory specimens for transport. Responsible for ensuring requested tests, patient counseling, immunizations, and ordered treatments are carried out within the confines of Licensed Vocational Nurse scope of practice. Responsible for medicating patients as directed by providers. Supervises follow-up activities such as consultation referrals and results of tests and x-rays ordered by providers. Provides clinical supervision of the medical assistants as needed and assists with patient care when needed. Assists practitioners with medical procedures as needed. Works collaboratively with front office, back office, and medical department staff to ensure timely rooming of patients, appropriate patient flow, and appropriate communication between the front and back office. Participates in case management of patients in conjunction with providers and the Community Services department, providing education and information to the patient, completing of necessary documentation, participating in processing referrals, and assisting patients in scheduling revisits to the clinic. Participates as a member of the Diabetes Care Team by attending regular meetings and providing necessary information on patients from the Diabetic Registry. This includes case management of all Native American/Alaskan Native diabetic patients, CenCal SMART program, and other patient panels as assigned. Responsible for learning and maintaining data entry into the electronic health record to record medical information. Adheres to Santa Ynez Tribal Health Clinic (SYTHC) practice guidelines; adheres to policies and procedures concerning OSHA, CLIA and all other staff and patient's safety issues. Maintains working knowledge of all programs that are provided such as CHDP/Patient Assistance Program, Medi-Cal, Healthy Families, Family PACT, Every Women Counts, etc. Acts as liaison with Community Services in requesting Purchased Referred Care (PRC) home visits as ordered by the physicians. exam is required. Valid Vocational Nursing License in the state of California. Valid CPR certification. Bilingual in English and Spanish are preferred. Intermediate computer proficiency utilizing Microsoft applications, e-mail, and internet. Willingness to work flexible schedules in a 24/7 work environment, including holidays, nights, and weekends.
