Job Description

Atterdag Village of Solvang is a Continuing Care Retirement Community, offering our residents independent, assisted living, memory care and skilled nursing care. Our Nursing staff is a team-oriented group, working together to provide the highest quality of care to our residents, proving that we are People Who Care. We currently have the following Full-Time position open on our team: Licensed Vocational Nurse Must pass state required physical, and TB test. Must be fingerprinted and free of criminal convictions. Must have graduated from an accredited school of nursing and successfully passed the California State Board of Licensed Vocational Nursing Exams. Must have active California Licensed Vocational Nursing License. Must have current CPR certificate. Must be able to communicate effectively in English, verbally and in writing, receive written and oral instructions and possess good interaction skills. Must adhere to department dress code. Must be able to safely perform essential job functions with or without reasonable.
Essential Job Functions and Responsibilities:
1. Admit, transfer and discharge residents Interacts with resident's physician, in-house (within the facility) and outside professionals, family members and significant others in order to acquire or convey pertinent resident information (including medical, nursing, psycho-social, emotional and spiritual) in order to develop an individualized plan of care for each resident. This is to assure both quality and continuity of care. Receives, documents and implements physicians' orders. 2. Coordinates individualized care for each resident Obtains physicians orders, documents appropriately on physician order sheet and implements orders in a timely manner for each resident within his/her assignment. Observes and records special nursing needs on Resident Care Plan and in resident's individual medical record. Includes resident's family in the care planning process. Conveys needs and care plan to CNA staff assigned to nurse's team. Initiates caregiving interventions and evaluates outcomes of plan of care, reporting success and/or need for change to attending physician (as appropriate), Director of Nursing and/or at Care Planning Meetings. Documentation of observation/changes is made in the resident's medical record. Informs/teaches staff under his/her supervision, any change/s in resident's plan of care. Coordinates nursing services and other resources within the facility and also uses or refers to community resources, as needed (e.g. contracted rehab services, hospice, psychological services, pastoral counseling, etc.). Demonstrates effective verbal and written communication skills with resident, family, professionals and staff. Documents weekly assessments in resident's medical record. 3. Medication Management Obtains orders, administers, observes for effectiveness and/or side effects of medications. Possesses judgment and skills to initiate immediate appropriate measures in response to untoward reaction to medication. Reports untoward effects to attending physician and documents events in resident's medical record. 4. Skilled procedures Possess the knowledge and technical skill to perform skilled nursing procedures, such as dressing changes and wound care, colostomy care, tracheotomy care, etc. Documents performance of skilled care, observes response to treatment, and reports any changes in caregiving needs, change in resident condition related to skilled care, or response to specific procedures to appropriate professionals (MD, Director of Nursing, other members of facility staff). Has the skill and ability to train new staff in caregiving and/or other required procedures. 5. Initiates caregiving interventions and evaluates their effectiveness. Demonstrates ability to evaluate resident caregiving needs, change of condition or response to treatment/care plan program and document appropriately, in addition to reporting to all appropriate parties (MD, family, DON, etc.) Teaches staff to observe, report, evaluate and record results of interventions 6. Participates in staff meetings, inservices and other professional programs. Is required to keep up-to-date with new/changed facility policies and procedures. Is required to update and/or develop new skilled nursing techniques and/or improve caregiving or supervisory skills in order to i. Provide resident care appropriate to plan of care and resident care needs. ii. Meet facility goals for providing quality and continuity of care. iii. Meet regulatory requirements to assist facility in achieving compliance with state and federal regulations. 7. Supervises and directs the care of support staff, including Certified Nursing Assistants (CNAs), Nurse Helpers and others assigned to his/her caregiving team. Instructs and aids staff with difficult procedures. Provides resident assignment and plan of care updates to support staff Participates in performance evaluation of support staff conducted by DON. Participates in support of college training programs, as appropriate, which use facility as training site. Other supervisory duties as designated by Director of Nursing. 8. Other duties as assigned.
