Job Description

CareOne Home Health is looking for an LVN for our growing Home Health business serving Tuolumne and Calaveras Counties. Mileage is reimbursed and driving time is paid. Requirements:
2 years LVN experience, home health preferred LVN license, required IV Therapy/Blood Draw certification required Proficient on all Microsoft Office software (Word, Excel, PowerPoint, Outlook) Experience with Kinnser (WellSky) EMR preferred Experience with Allscripts preferred Primary responsibilities will be: Responsible for the oversight and delivery of quality, patient-centered care rendered in the patient's residence. Follow the patient Plan of Care (POC) as directed by RN Case Manager and PCP Communicate necessary revisions to patient status and/or needs to RN Case Manager and Clinical Manager when applicable Regularly consult, advise and reevaluate patient status with patient, family, patient representative and other team members as appropriate regarding safety, plan and outcome of care and discharge planning. Provide complete, accurate and timely documentation consistent with the plan of care
