M-F | 8am- 4:30pm |40hr/ week Job Summary Under the direction of the Provider and Clinical Nurse Manager, the LVN/LPN will provide direct nursing care and care coordination support for elderly and frail participants both within the clinic and in community settings. This role includes wound care, medication administration, participant monitoring, care coordination, and collaboration with interdisciplinary teams to ensure high-quality patient-centered care. Responsibilities Provide direct nursing care and support to elderly and frail participants in both clinic and community settings under the direction of the Provider and Clinical Nurse Manager Perform and document wound care, medication administration, point-of-care testing, and other ordered clinical procedures within scope of practice Support medication management including medication reconciliation, specialty pharmacy coordination, medication education, and addressing participant medication concerns Coordinate medication updates and follow-up care related to procedures, specialist visits, and provider recommendations Conduct pre-operative, post-operative, high-risk monitoring, and proactive participant follow-up touchpoints while escalating concerns appropriately to the care team Monitor participant conditions closely and communicate any status changes to Providers, RN Case Managers, and interdisciplinary team members Serve as a liaison between participants, caregivers, providers, and support networks to ensure continuity of care Maintain accurate and timely documentation while ensuring HIPAA compliance and participant confidentiality Participate in staff trainings, quality improvement initiatives, meetings, and organizational activities Communicate effectively with supervisors and team members regarding workload, priorities, and participant care needs Adapt to evolving business needs and perform additional duties as assigned Required Qualifications Graduate of an Accredited School of Nursing Active and unencumbered LVN/LPN License Minimum 1 year of nursing experience in a clinical setting with elderly or frail populations Strong clinical judgment and organizational skills Ability to work independently in a fast-paced environment Excellent communication and interpersonal skills Experience working with diverse populations and interdisciplinary teams Preferred Qualifications Experience working with senior or vulnerable populations preferredSimilar jobs in Stockton, CA
