Role:
The Licensed Practical Nurse (LPN) plays a critical role in delivering high-quality nursing care within home health settings, including private homes and/or chosen place of residents. This position focuses on providing direct patient care, managing wound care, administering medications, and performing essential clinical procedures such as venipuncture. The LPN will also educate patients and their families on health management and pain control strategies to promote recovery and well-being. Collaborating closely with Nurse Practitioners and other healthcare professionals, the LPN ensures comprehensive and coordinated care tailored to each patient's needs. Ultimately, the role aims to enhance patient outcomes through compassionate care, effective communication, and adherence to nursing standards and regulations.
Minimum Qualifications:
Valid and current Nursing License (LPN) in California. Proficiency in reading, writing, and speaking English to effectively communicate with patients and healthcare teams. Demonstrated experience in patient care and wound management. Ability to perform venipuncture and basic clinical procedures safely and accurately. Knowledge of pain management principles and patient education techniques.
Preferred Qualifications:
Experience working in a home health setting or similar healthcare environment. Previous collaboration experience with Nurse Practitioners or advanced practice providers. Additional certifications related to wound care or pain management. Familiarity with electronic health record (EHR) systems. Strong interpersonal skills and cultural competence in diverse patient populations.
Responsibilities:
Provide direct patient care including monitoring vital signs, administering medications, and assisting with daily living activities. Perform wound care management, including cleaning, dressing, and monitoring healing progress. Conduct venipuncture and other basic clinical procedures as required. Educate patients and their families on health conditions, treatment plans, and pain management techniques. Collaborate with Nurse Practitioners and healthcare teams to develop and implement individualized care plans. Maintain accurate and detailed patient records in compliance with healthcare regulations. Ensure a safe and supportive environment within assisted living facilities or clinical settings. Respond promptly to patient needs and communicate changes in condition to appropriate medical staff.
