Job Description

LVN Case Managers - Visit Nurse. $35 per visit. This position offers 4 visits a day, 2 days a week. A Licensed Vocational Nurse (LVN) visit nurse provides skilled nursing care to patients in their homes, following a care plan established by a registered nurse or physician. Their duties include monitoring vital signs, administering medications, providing wound care, assisting with daily living activities, and educating patients and their families about care. They also document patient care and communicate with the healthcare team about patient status. Key Responsibilities of an
LVN Visit Nurse:
Patient Assessment and Care:
Assessing patients' physical, mental, and social status, documenting findings, and reporting changes in condition to the supervising nurse or physician.
Medication Administration:
Administering medications as prescribed, including injections, and monitoring for any adverse reactions.
Wound Care:
Providing wound care, changing dressings, and ensuring proper wound healing. Assistance with
Daily Living:
Helping patients with tasks such as bathing, dressing, and eating, promoting independence and comfort.
Education and Training:
Educating patients and their families on disease management, medication administration, and other self-care activities.
Communication and Collaboration:
Communicating with the supervising nurse, physician, and other members of the healthcare team regarding patient progress and needs.
Documentation:
Accurately documenting all care provided, observations, and communications in the patient's medical record.
Infection Control:
Following infection control protocols to maintain a safe environment for both the patient and the nurse.
Emergency Response:
Recognizing and responding to emergency situations, escalating care as needed.
Home Health Specific:
Depending on the agency, some LVNs may also be involved in tasks like medication reconciliation, coordination of care with other services, and providing emotional support to patients and families.
