If you feel called to service, we would love to have you on our team. GIMAG HealthCare is seeking dynamic and highly motivated Clinicians to join our team. We currently provide Home Health patient care in Alameda, Contra Costa, Counties. As a Hospice or Home Health LVN, you will provide nursing care and supportive services in accordance with the physician/provider treatment plan and the nursing process of assessment, diagnosis, planning, implementation and evaluation under the supervision of the Registered Nurse. Performs accurate assessments, interprets and correlates findings, establishes plan of care, observes patient progress, records pertinent observation, implements physician and nursing treatment plans and evaluates outcomes. Responsibilities and Duties Ability to complete all documentation in accordance with policy and procedure. Flexible, as varied daily activities will include phone, email and written communication, meetings with a variety of healthcare professionals, great understanding of hospice rules and regulations Must work closely with team members to provide the optimal care and services. Ability to manage scheduling visits, documentation time, and meetings effectively. Ability to learn an electronic medical records system. Qualifications and Skills Licensed Vocational Nurse License in California 1 year Professional Experience Home Health Experience preferred Current CPR Certification Ability to function autonomously Strong clinical skills Effective communication skills Proficient computer skills Reliable transportation and insurance
