Job Description

Job Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) / Resident Care Coordinator (RCC) to join our dynamic healthcare team. In this vital role, you will provide high-quality nursing care, coordinate resident services, and ensure a safe, comfortable environment for our residents. Your expertise will support individualized care plans, promote health and wellness, and foster a positive community atmosphere. Responsibilities Deliver comprehensive nursing care, including medication administration, vital signs monitoring, and physical assessments Develop, implement, and update personalized care plans in collaboration with interdisciplinary teams Coordinate resident activities related to health management, including IV infusion, catheter care, and wound management Conduct triage assessments to determine urgency and appropriate interventions for residents' health concerns Maintain accurate medical documentation using electronic health record (EHR) systems such as Epic or Cerner Ensure compliance with HIPAA regulations and uphold patient confidentiality at all times Provide education and support to residents and their families regarding health conditions, medications, and treatment options Assist with discharge planning, post-acute care coordination, and referrals to specialists when necessary Qualifications Valid state licensure as a Licensed Practical Nurse (LPN) or equivalent certification Proven experience in long-term care, assisted living, or nursing home environments Strong knowledge of medical terminology, anatomy, and physiology Experience with EMR/EHR systems preferred Skills in patient assessment, medication administration, catheterization, and sterile techniques Ability to work effectively with individuals with dementia-related conditions such as Alzheimer's disease or memory care needs Excellent communication skills Knowledge of infection control protocols and safety procedures in healthcare environments Join our team to provide compassionate care that transforms lives!
