Job Description:

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team. The ideal candidate will have a strong commitment to providing high-quality care, excellent clinical skills, and a desire to work in a collaborative team environment.

Responsibilities:

• Provide direct nursing care to residents in accordance with their care plans
• Administer medications and treatments as prescribed
• Monitor and document residents' health status and responses to treatments
• Assist in the development and implementation of individualized care plans
• Collaborate with healthcare team members to ensure optimal resident care
• Educate residents and their families on health-related issues and care practices
• Maintain accurate and timely documentation in compliance with facility policies and regulations
• Participate in continuous quality improvement activities

Qualifications:

Valid and active LPN license in the state of Colorado Current CPR certification Experience in a skilled nursing or long-term care setting preferred Strong clinical and assessment skills The Gardens Skilled Nursing and Rehab is an equal opportunity employer.
