Job Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. In this vital role, you will provide high-quality patient care across various settings, including long-term care, assisted living, hospitals, and community health environments. Your expertise will support patient recovery, comfort, and well-being through skilled nursing interventions, medication administration, and thorough assessments. Responsibilities Deliver comprehensive nursing care by performing physical examinations, vital signs monitoring, and patient assessments with precision and empathy. Administer medications, injections, IV infusions, and treatments such as wound care, catheterization, and tube feeding following established protocols. Utilize electronic health record (EHR) systems like Epic or Cerner to document patient information accurately and maintain detailed medical records in compliance with HIPAA regulations. Conduct triage assessments to prioritize patient needs effectively in urgent care or emergency settings such as Level I Trauma Centers or emergency rooms. Support patients with complex conditions including Alzheimer's disease, dementia, infectious diseases, and chronic pain management through specialized care plans. Collaborate with multidisciplinary teams to develop discharge planning strategies and coordinate post-acute or home care services. Engage in ongoing education on ICD coding (International Classification of Diseases), ICD-9/10 updates, and medical terminology to ensure precise documentation and billing accuracy. Assist with diagnostic evaluations using medical imaging or laboratory specimen collection when necessary. Provide compassionate care for diverse populations including pediatrics, geriatrics, neonatal ventilator patients, individuals with developmental disabilities, and those requiring hospice or palliative care. Maintain aseptic techniques during procedures such as suturing or sterile processing to prevent infections and ensure patient safety. Qualifications Valid state licensure as a Licensed Practical Nurse (LPN) with current certification. Proven experience in critical care environments such as ICU or Level II trauma centers is highly desirable. Demonstrated proficiency with EMR/EHR systems like Epic, Athenahealth, or eClinicalWorks for accurate documentation. Strong knowledge of anatomy, physiology, pharmacology, and medical terminology relevant to diverse clinical settings. Experience working with individuals with disabilities or special needs including Alzheimer's care and memory support programs. Skilled in medication administration techniques including injections, venipuncture, catheter care, and IV insertion/infusion procedures. Ability to perform physical examinations and vital signs assessment efficiently while demonstrating excellent patient communication skills. Knowledge of infection control practices and aseptic techniques essential for maintaining a safe environment. Familiarity with ICD coding (ICD-9/10), CPT coding for procedures like spinal taps or gastric lavage is advantageous.
