36 hours/week Job Summary Red Cliffs Post Acute is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our Long-Term Care and Skilled Nursing team. The LPN will provide direct patient care, administer medications and treatments, monitor patient conditions, and support residents in maintaining the highest possible quality of life in a post-acute and long-term care setting. Key Responsibilities

• Deliver high-quality nursing care to residents in accordance with care plans and physician orders
• Administer medications, treatments, and injections safely and accurately
• Monitor and document patient conditions, vital signs, and changes in health status
• Collaborate with Registered Nurses, physicians, therapists, and interdisciplinary team members
• Assist with admissions, discharges, and transfers
• Maintain accurate and timely patient records and documentation
• Support residents and families with education and compassionate communication
• Ensure compliance with all facility policies, state regulations, and infection control standards Qualifications
• Current LPN license in the state of Colorado
• Strong clinical assessment and communication skills
• Ability to work effectively in a fast-paced healthcare environment

Pay:

$1,700.00 - $1,800.00 per week
