We are currently seeking a highly qualified LPN to join our growing healthcare team for part-time work, offering pay from $40 to $ 47 per visit, and offering flexible schedules. We provide the highest quality of home care in the Denver, Elbert, Arapahoe, Adams, Douglas, Jefferson, Gilpin, El Paso, Boulder, Larimer, Clear Creek, Weld area. Key Responsibilities:
Provide direct nursing care as outlined in the client's individualized care plan. Administer medications and treatments as prescribed by a physician or RN supervisor. Monitor and record vital signs, symptoms, and changes in client condition, reporting concerns promptly to the RN or physician. Perform wound care, catheter care, and other skilled nursing tasks within the scope of practice. Assist with personal care activities, mobility, and activities of daily living (ADLs) as needed. Support clients in managing chronic conditions through education and consistent care. Document care provided accurately and promptly, following agency and state guidelines. Collaborate with RNs, caregivers, social workers, and other members of the care team to ensure coordinated services. Provide emotional support to clients and families, promoting comfort and reassurance. Adhere to infection control practices, safety standards, and all regulatory requirements.
Qualifications:
Current and active LPN license in Colorado. Minimum of two years of nursing experience; home health or long-term care experience preferred. Knowledge of basic nursing practices, medication administration, and chronic disease management. Strong observation and documentation skills. Compassionate, patient, and dependable with excellent communication skills. Ability to work independently and as part of a team. Ability to assist clients with mobility and personal care as needed. Reliable transportation, a valid driver's license, and car insurance to travel to client homes.
