Duties and Responsibilities include the following, but not limited to: The RN is responsible for being on call, as assigned and agreed to, to answer any medical or medication questions/concerns
  • Teach QMAP class once a week or as needed
  • Reviews and makes necessary input on GERs, T-Logs, MARs, health tracking, etc.
  • Assist case managers in interpretation and processing medical appointments, annuals, and consultation forms
  • Enters and adjusts medications for incoming and established individuals that ILC supports
  • Monthly MAR approvals for individuals we support
  • Assist with monthly MAR audits and data checks on MAR's
  • Maintain appointments, inputs caseloads, responds to scomms, and other maintenance as needed in Therap
  • Acts as liaison between Physician offices, pharmacies, ILC team members and nurses
  • Serves on Quality Improvement and Therap Committees.
  • Create and update healthcare plans and protocols when entering services and as medical conditions change
  • Performs quality improvement activities and on-site visits on a semi-annually basis
  • Attends IDT meetings and team meetings when a person supported medical needs may require more attention.
  • Works a minimum of 40 hours a week.
  • Trains on G-tube competency and provides required G-tube monitoring
  • Conducts staff trainings on medical requirements/needs
  • Performs all other duties assigned

Language Ability:
Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
Math Ability:
Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumference, and volume. Ability to apply concepts of basic algebra and geometry.
Reasoning Ability:
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
Computer Skills:
To perform this job successfully, an individual should have knowledge of word processing software, spreadsheet software and Therap. Ability to type emails and reports quickly.
Education/Experience:
Graduate of a state-approved school of professional nursing. Experience working with the IDD population preferred. Successfully complete and pass all checks including:
Colorado Adult Protective Services Central Registry Child Protective Services Central Registry Colorado State Patrol Criminal Background Check Federal Bureau of Investigation Background Check Certificates and Licenses:
Valid Driver's License Basic life support certificate Active Registered Nursing. You may start with a compact license, but it is expected to have a Colorado issued license within a year.
