Pay Range:
$30.15 - $36.40 per hour based on experience
Schedule:

Current Practical Nurses License (L.P.N.), Cardiopulmonary Resuscitation (CPR) certification required. A minimum of one year's experience in working with the frail or elderly population required. Intravenous Venous (IV) Certification preferred. Must have a valid driver's license, proof of insurance and have means of transportation. What You'll Be Responsible For Responsible for processing of medication orders for all participants through direct input or processing of medication orders input by staff provider, ensuring that a complete, accurate, and legal prescription is received by the dispensing pharmacy. Responsible for coordination to maintain participant medication schedules (maintenance of cycle system) through implementation of new medication orders, mid-cycle medication changes, discontinuations, and reconciliations through transition process as prescribed by policy. Responsible for medication reconciliation through care setting transitions (LTC/Hospital/ALF discharge) for all participants in accordance with organization procedures and facilitates any changes required. As a Program of All-Inclusive Care for the Elderly (PACE) provider and the first of its kind in Western Colorado, our interdisciplinary teams deliver personalized, comprehensive care tailored to each participant's needs.
