Up to $49/hr (based on experience) Position Overview "Navitas Healthcare, LLC" is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join a dedicated long-term care and skilled nursing team in Grand Junction, Colorado. The ideal candidate will possess strong clinical and patient care skills, with experience in long-term care, rehabilitation, or post-acute settings preferred. This position requires flexibility for night shifts and the ability to thrive in a fast-paced healthcare environment while delivering exceptional resident-centered care. Key Responsibilities Provide direct nursing care in accordance with individualized care plans and physician orders Administer medications and treatments safely and accurately Monitor, assess, and document resident conditions and report any significant changes Perform routine nursing procedures including wound care, vital signs, and basic assessments Assist residents with activities of daily living (ADLs) while ensuring dignity, comfort, and safety Collaborate with nurses, providers, caregivers, and interdisciplinary teams to support quality patient outcomes Maintain accurate and timely clinical documentation Adhere to infection prevention, safety, and regulatory guidelines Support care planning initiatives and resident-focused treatment goals Respond promptly and professionally to resident needs during assigned shifts Qualifications Active Licensed Practical Nurse (LPN) license in the state of Colorado required Graduate of an accredited Practical Nursing program Previous experience in long-term care, skilled nursing, rehabilitation, or post-acute care preferred Current BLS/CPR certification preferred Strong clinical, assessment, and documentation skills Ability to work independently and collaboratively within a healthcare team Excellent communication and interpersonal skills Compassionate, dependable, and patient-focused approach to care Ability to manage multiple patient assignments efficiently in a fast-paced environment For more details contact at hdavda@navitashealth.com or Call / Text at 516-862-1169.
