In this vital role, you will provide high-quality patient care across various settings, including long-term care facilities, assisted living communities, hospitals, and outpatient clinics. Your expertise will support patient recovery, comfort, and well-being through skilled nursing interventions. You will work closely with interdisciplinary teams to ensure comprehensive care delivery while adhering to all safety and regulatory standards. This position offers an exciting opportunity to make a meaningful difference in patients' lives by applying your clinical skills and compassionate approach. Responsibilities Deliver direct patient care by performing physical examinations, vital signs assessment, and patient evaluations using your physiology knowledge and clinical judgment. Administer medications, injections, IV infusions, and perform catheterizations following sterile technique and aseptic procedures. Assist with complex medical procedures such as tube feeding, wound suturing, spinal taps, and ventilator management in various settings including ICU, emergency room, or outpatient clinics. Document patient information accurately using EMR (Electronic Medical Records) systems like Epic, Cerner, or Athenahealth while ensuring compliance with HIPAA regulations. Conduct patient triage in urgent care or emergency medicine environments to prioritize care based on acuity levels such as Level I Trauma Center or Level II Trauma Center experience. Collaborate with physicians and other healthcare professionals in discharge planning, case management, and post-acute care coordination for diverse populations including geriatrics, pediatrics, neonatal patients, or individuals with developmental disabilities. Participate in infection control practices and assist with specimen collection/processing to support diagnostic evaluation and infection prevention efforts. Qualifications Valid license as a Licensed Practical Nurse (LPN) in the relevant state or jurisdiction. Proven experience in critical care settings such as ICU or emergency room environments; hospital medicine experience preferred. Familiarity with EMR/EHR systems like Epic, Cerner, or Athenahealth for accurate medical documentation. Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/ICD-10), CPT coding, and medication administration protocols. Experience working with diverse patient populations including seniors in geriatrics or memory care units; dementia and Alzheimer's care experience is highly valued. Ability to perform venipuncture, phlebotomy, sterile procedures such as catheter care or spinal taps; proficient in basic life support (BLS). Excellent patient assessment skills combined with compassionate communication to support patients' physical and emotional needs across various healthcare environments such as home & community care or outpatient clinics.
