Per Diem SUMMARY:
Provides nursing care to assigned Residents in accordance with the policies and core values of Mozaic Senior Life. Practices within the scope of the role of the RN. Responsible for assisting the Salsa in the overall evaluation, implementation, delivery and coordination of Resident care services including assisting in monitoring the medication management program.

Certified Nursing Assistants (Per Diem)
Schedule:
Per Diem 7am-3pm & 3pm-11pm shifts available. (Weekend shifts is the current need) DUTIES
Essential Job Functions:
1.Assists in supervising the medication program including clarifying, transcribing and carrying out physician orders, performing medication management evaluations, administering medication and treatment, and provides teaching to Residents. 2.Performs Resident assessments and implements service care plans. 3.Responds to and provides emergency care within guidelines as needed. 4.Documents accurate information in a timely, concise manner as required per State regulations and the Community. 5.Communicates and collaborates effectively with Residents, families, colleagues and other members of the assisted living team to meet Resident and household needs. 6.Maintains positive relations with Residents, families and physicians. 7.Reports any significant incidents or changes with Resident's needs to Executive Director or Salsa. 8.Supervises the CNA's in implementing, delivering and coordinating service care plans to assure compliance with the quality of care. 9.Assists Residents with Activities of Daily Living as needed. 10.Assists with the training of CNA's and participates in orientation of new staff. 11.Participates in educational programs, in-services, meetings and workshops and utilizes current literature to enhance professional skills and knowledge. 12.Participates in quality improvement programs. 13.Adheres to the policies and procedures of Jewish Senior Services. Maintains confidentiality at all times. 14.Successfully completes the Medication Management training and the Core Competencies for licensed nurses. 15.Works weekends and holidays on a rotating basis according to the needs of the department. 16.All other duties as assigned.
QUALIFICATIONS
Minimum Qualifications:
Education:
Graduate from an accredited school of nursing
Experience:
1+ years clinical experience in a hospital, home care or similar health care setting
Licensure:
RN/LPN , State of Connecticut, CPR certification
