• What You'll Do
• + Deliver skilled nursing care under RN/physician direction + Follow and implement individualized patient care plans + Monitor patient conditions and report changes promptly + Educate patients and families on self-care and safety + Administer medications and maintain accurate documentation + Collaborate with RNs and interdisciplinary care teams + Help prevent avoidable hospitalizations through proactive care •What You'll Need•+ Active LPN/LVN license (state required) + Graduate of an accredited nursing program + 1+ year of clinical nursing experience + Home Health or Hospice experience (preferred) + Ability to travel throughout the East Hampton coverage area (~50%) + Reliable transportation, valid driver's license, and auto insurance + Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs •Ready for a Role That Feels Different?
