Position:
Disability Resource Network, Inc. (DRN) is seeking a compassionate and organized Licensed Practical Nurse (LPN) to support our DDS-funded Adult Day Program in Derby, CT. The program serves approximately 30 individuals age 22 and older with developmental and, at times, physical disabilities. This role is not strictly clinical in nature. It functions similarly to a "school nurse" position, with a strong focus on medical documentation, preventative care, minor first aid, illness response, and coordination of care. The LPN will work under the supervision and license of a Registered Nurse (RN), who is on call 24/7, conducts regular site visits, reviews and signs documentation, performs medication observations, and provides annual OSHA training. Duties may be delegated in accordance with DDS guidelines. Primary Responsibilities Manage and maintain participant medical files in compliance with DDS and state regulations Send, receive, and track bi-annual Physician's Orders Obtain and manage medical protocols, including (but not limited to): Seizure Protocols Diabetic Protocols EpiPen Protocols Other specialized medical directives Conduct annual Fall Risk Assessments and develop individualized Fall Risk Plans Review participant records and assign appropriate medical protocols Serve as liaison between program staff, RN supervisor, physicians, pharmacies, and caregivers Complete required documentation and follow-up as directed by the supervising RN Perform medication "pass and pour" in accordance with DDS regulations Administer insulin and support diabetic care management Administer medications to program participants Provide minor first aid and respond to participant illness or injury occurring during program hours Monitor general health concerns and communicate changes appropriately Assist with general administrative duties during downtime (filing, answering phones, record organization, etc.) Minimum Qualifications Current Connecticut Licensed Practical Nurse (LPN) license in good standing Experience working with individuals with developmental disabilities preferred Experience in medication administration and insulin administration required Strong organizational and documentation skills Ability to work independently while collaborating with interdisciplinary team members Knowledge of DDS regulations preferred Work Environment & Schedule Adult Day Program setting Monday through Friday Standard hours: 8:30 AM - 3:30 PM Opportunity for expanded schedule (8:00 AM - 4:00 PM, up to 40 hours/week) based on program needs Supportive, team-oriented environment Employee Benefits As an employee of Disability Resource Network, Inc., you are eligible for: Weekly pay via Direct Deposit or paper check Health Insurance Dental Insurance Vision Insurance Life Insurance Long-Term Disability Insurance 401(k) plan with 3% employer match Employee Discount Program Employee Assistance Program (EAP)
