Licensed Practical Nurse (LPN) - Newington, Connecticut $2,000 Sign On Bonus for Second Shift!! Join the Civita Care Center at Newington team and be part of a unique care experience that sets the standard for sub-acute rehabilitation in our community. As a Licensed Practical Nurse (LPN) with our organization, you will have the opportunity to deliver exceptional care to our residents while collaborating with a multidisciplinary team of healthcare professionals. About the
Role:
As an LPN at Civita Care Center at Newington, you will be responsible for providing high-quality patient care to our residents, under the supervision of our licensed nurses and physicians.
You will be expected to:
Assess patient needs and develop personalized care plans that address their unique requirements Administer medications and treatments as prescribed by physicians Provide wound care and maintenance of complex medical equipment Conduct vital sign checks and report any changes or concerns to the nursing staff Participate in quality improvement initiatives to ensure the highest standards of patient care
Responsibilities:
Collaborate with the interdisciplinary team to develop and implement individualized care plans Provide direct patient care, including but not limited to: + Vital sign checks + Medication administration + Wound care + Medical equipment maintenance Maintain accurate and detailed records of patient care and progress Participate in quality improvement initiatives and contribute to the development of policies and procedures Demonstrate excellent communication and interpersonal skills when interacting with patients, families, and healthcare staff
