Full time 3-11p About Villa Maria Villa Maria Rehabilitation & Nursing, a well-respected Short-Term Rehabilitation and Skilled Nursing Facility located in Plainfield, CT, is seeking a dedicated and compassionate Charge Nurse (RN/LPN) to join our clinical care team. Position Overview The LPN Floor Nurse / Charge Nurse provides direct nursing care to residents and supervises the day-to-day nursing activities of assigned staff on the unit. This position works under the direction of the Registered Nurse Supervisor and ensures that the highest quality of care is consistently provided in accordance with facility policies, professional standards, and regulatory requirements. Responsibilities Accurately record and maintain residents' medical histories, progress notes, and treatment records. Administer medications and treatments as ordered by physicians and according to facility protocols. Observe and monitor residents' conditions, behaviors, and responses to medications or treatments; promptly report changes to the supervising RN and physician. Monitor residents' fluid intake and output and document all observations accurately. Administer injections, dress wounds, collect lab specimens, and perform other direct care tasks as assigned. Supervise and support assigned CNAs and other nursing personnel to ensure compassionate, safe, and efficient care delivery. Provide emotional and psychological support to residents and their families. Communicate effectively with residents' family members, providing education, comfort, and updates on care plans as appropriate. Ensure compliance with infection control, safety, and confidentiality policies. Maintain accurate and timely charting in PointClickCare (PCC) and other electronic record systems. Exhibit professionalism, teamwork, and compassion in all interactions. Qualifications Valid Connecticut LPN or RN license in good standing. Minimum of 1 year of experience in a long-term care or skilled nursing setting preferred. Strong clinical skills with the ability to assess, plan, implement, and evaluate resident care. Working knowledge of Microsoft Office and PointClickCare (PCC) preferred. Excellent communication and leadership skills. Compassionate, dependable, and able to manage stress effectively in a fast-paced environment.
