25 Hours Every Other Week Position Overview Rocky Hill Veterans Home & Hospital is seeking a compassionate and dependable Licensed Practical Nurse (LPN/LVN) to provide high-quality nursing care to veteran residents. The ideal candidate will be committed to supporting the health, safety, and well-being of residents while working collaboratively with an interdisciplinary healthcare team. Responsibilities Provide direct nursing care to residents in accordance with physician orders and care plans. Administer medications and treatments accurately and safely. Monitor resident conditions and promptly report changes to the Registered Nurse and healthcare team. Assist with resident assessments, care planning, and implementation of nursing interventions. Maintain accurate, timely, and thorough documentation in medical records. Promote resident comfort, dignity, and quality of life. Follow facility policies, infection control standards, and state regulations. Collaborate with nurses, physicians, and other healthcare professionals to ensure exceptional patient care. Qualifications Active Connecticut Licensed Practical Nurse (LPN) license in good standing. Current CPR/BLS certification preferred. Previous experience in long-term care, skilled nursing, rehabilitation, or hospital settings preferred. Strong communication, organizational, and documentation skills.
