Job Summary We are seeking a motivated and detail-oriented Licensed Practical Nurse (LPN) / Assistant Director of Nursing (Assistant DON) to support the clinical and administrative operations of our home healthcare agency in Washington, D.C. The ideal candidate will assist with maintaining patient care plans, clinical documentation, compliance requirements, staff coordination, and communication with patients, caregivers, nurses, physicians, and payers. This position requires strong computer skills, knowledge of home healthcare documentation, and the ability to navigate D.C. Care Connect and comply with regulations set forth by the Department of Health Care Finance (DHCF) . New LPNs who are organized, willing to learn, and comfortable with computer-based documentation are encouraged to apply. Responsibilities The LPN / Assistant DON will assist with: Reviewing and maintaining patient clinical records and plans of care Navigating and documenting through D.C. Care Connect Ensuring compliance with D.C. home healthcare regulations and DHCF requirements Assisting with Start of Care (SOC) , Resumption of Care (ROC) , recertifications , and ongoing patient documentation Reviewing and organizing 485 Plan of Care documents Processing and tracking physician orders, medication changes, and clinical updates Ensuring medications, diagnoses, services, frequencies, goals, and interventions are accurately reflected in the plan of care Communicating with physicians, case managers, patients, caregivers, and clinical staff Assisting with quality assurance and clinical chart audits Helping develop, update, and maintain agency protocols, policies, procedures, and internal workflows Tracking due dates for recertifications, orders, authorizations, and required clinical documentation Supporting nurses and aides with documentation questions and care coordination Maintaining accurate records using Microsoft Excel and other computer systems Assisting the DON with daily clinical operations and regulatory readiness Helping ensure that services provided match authorized and documented care needs Qualifications Active LPN license in Washington, D.C. or eligibility to obtain D.C. licensure Interest or experience in home healthcare, long-term care, skilled nursing, or community-based care Knowledge of or willingness to learn:

Plan of Care Recertifications Physician orders Medication documentation Care plan development Home healthcare compliance Ability to navigate D.C. Care Connect or willingness to be trained Strong computer skills required Proficiency with Microsoft Excel, Word, email, scanning, uploading documents, and electronic recordkeeping Ability to write and update protocols, procedures, and clinical documentation Strong attention to detail and ability to meet deadlines Strong communication and organizational skills Ability to work independently and as part of a clinical team New LPN graduates are welcome to apply Preferred Qualifications Prior home healthcare experience Familiarity with DHCF requirements Experience with Medicaid home healthcare documentation Experience reviewing care plans, authorizations, orders, and clinical records Bilingual ability is a plus but not required Skills Needed Strong documentation and chart review skills Ability to manage multiple deadlines Strong computer and data-entry skills Ability to use Excel for tracking, auditing, and reporting Professional communication with patients, staff, physicians, and case managers Willingness to learn home healthcare regulations and agency procedures Work Environment This position is primarily office-based with possible field-related responsibilities depending on agency needs. The LPN / Assistant DON will work closely with the Director of Nursing, administrative staff, field nurses, aides, patients, and caregivers.
