Our HEP-C Program:

Act as a liaison between patients, providers, insurance companies and pharmacies, and assists the Nurse Care Manager with obtaining Prior Authorizations Follows individuals enrolled in treatment to ensure compliance and a smooth process through treatment, provides education and guidance under the direction of the Nurse Care Manager Provides education on Hep C, including risk of reinfection, possible side effects of medications, etc. Administers, reads and documents hepatitis C antibody testing and

Nurse Navigator LPN:

Clinical expertise: Understanding insurance processes and pharmacy protocols. Strong foundation in nursing practice, understanding of hepatitis C diagnosis and treatment, or willingness to learn. Ability to perform POC tests, experience in venipuncture, fingerstick and injections, IM, SQ. Data and technology : Proficiency in EMR, ability to complete and create spreadsheets. Communication skills and cultural competence : Skilled in cross cultural communications to support diverse patient populations with empathy and respect. Ability to provide conflict resolution. Positive attitude and ability to work in teams Organizational skills Good communication skills

REQUIRED

LPN

• Licensed Practical Nurse
• in the State of Delaware

REQUIRED

: 1-2 Years of Nursing Experience

PREFERRED

2-3 Years Healthcare/Medical Experience with Substance Abuse/Addiction Medicine/Infectious Diseases

PREFERRED

Experience in the field of Drug and Alcohol treatment

REQUIRED

: Valid Driver's License With 2 Points or Less

REQUIRED

From $30/hour Commensurate with experience, education, and certification / licensure!
