We are seeking a dependable, compassionate, and experienced Licensed Practical Nurse (LPN) to provide high-quality in-home nursing care to patients throughout Boca Raton and the surrounding communities . This is an excellent opportunity for an LPN who enjoys working independently while making a meaningful difference in patients' lives.

Responsibilities:

Provide skilled nursing care in patients' homes under the supervision of a Registered Nurse. Perform patient assessments and document clinical findings accurately. Administer medications and treatments as ordered by the physician. Monitor and report changes in patients' conditions. Educate patients and family members on disease management and care plans. Maintain timely, accurate, and compliant clinical documentation. Communicate effectively with physicians, case managers, and the interdisciplinary care team.

Required Qualifications:

Current Florida Licensed Practical Nurse (LPN) license is in good standing. Minimum of one year of nursing experience (home health experience preferred). Current CPR certification. Valid Florida driver's license, reliable transportation, and current auto insurance. Strong clinical, organizational, and communication skills. Ability to work independently and manage a flexible daily schedule. Experience with electronic medical records is a plus. Must be proficient in the understanding and performance of wound care.
