Job Summary Join our dedicated healthcare team as a Licensed Vocational Nurse (LVN) specializing in private duty nursing! In this vital role, you will provide personalized, high-quality nursing care directly to patients in their homes or private settings. Your expertise will help improve patient outcomes, promote independence, and support families through compassionate, skilled care. Duties Deliver comprehensive nursing assessments, including vital signs, physical examinations, and patient history evaluations to determine individualized care plans. Administer medications, injections, IV infusions, and treatments such as tube feeding or catheter care while adhering to all safety protocols and HIPAA regulations. Monitor patients' health status using advanced EMR (Electronic Medical Records) systems like Epic or Cerner to document medical records accurately and efficiently. Manage complex cases involving dementia care, Alzheimer's care, memory care, and patients with developmental disabilities or behavioral health needs. Provide specialized interventions such as wound care, IV insertion, sterile processing techniques, and acute pain management for diverse patient populations. Collaborate with physicians, case managers, and other healthcare professionals to coordinate discharge planning, post-acute care, or hospice services. Educate patients and families on health coaching topics including medication administration, nutrition, infection control, and disease management strategies. Qualifications Valid state licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN). Proven experience in critical care environments such as ICU or Level I Trauma Centers is highly desirable. Demonstrated proficiency with EMR/EHR systems like Epic, Athenahealth, or eClinicalWorks for accurate documentation. Experience working with individuals with disabilities or chronic illnesses including Alzheimer's disease and dementia is preferred. Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/ICD-10), and diagnostic evaluation techniques. Ability to perform physical examinations, venipuncture (blood draws), phlebotomy, and sterile procedures confidently. Excellent patient assessment skills combined with compassionate communication to support diverse populations including pediatrics and geriatrics.
