Matrix Home Care is seeking compassionate and skilled Licensed Practical Nurses (LPNs) to provide quality patient care in the home setting. This role supports a variety of service lines including skilled nursing visits, care management support, and hourly patient care. The ideal candidate is flexible, reliable, and comfortable working independently in the field.

Key Responsibilities:

Provide direct patient care in the home setting under the supervision of an RN Perform skilled nursing tasks such as wound care, medication administration, and monitoring vital signs Assist with ongoing patient assessments and report changes in condition Support care plan implementation and documentation Provide patient and caregiver education as directed by the RNC complete accurate and timely documentation in EMR system (KanTime preferred)Assist with admissions, follow-up visits, and continuity of care Ensure compliance with state regulations and company policies. Under Florida law, this role is subject to background screening per Chapter 435, Florida Statutes, including fingerprinting and review for eligibility before employment.
