  • Full-time Position Summary The Licensed Practical Nurse (LPN)
  • Telephone Triage Nurse provides clinical support and patient guidance over the phone.
This role involves assessing patient symptoms, determining urgency of care needs, providing health education, documenting calls, and coordinating follow-up care. Key Responsibilities Answer incoming patient calls and assess symptoms using established triage protocols. Collect and document patient health information accurately in the electronic medical record (EMR). Prioritize patient concerns and determine appropriate next steps based on clinical guidelines. Provide patients with home care instructions, medication guidance, and appointment scheduling recommendations. Escalate urgent or emergent cases to physicians, nurse practitioners, or emergency services as appropriate. Coordinate communication between patients, providers, pharmacies, and clinical staff. Monitor voicemail and callback requests in a timely manner. Maintain patient confidentiality and comply with HIPAA regulations. Support quality improvement initiatives and maintain accurate documentation standards. Participate in team meetings, training, and continuing education activities.
