In this vital role, you will provide high-quality patient care across various settings, including long-term care, assisted living, hospitals, and community health environments. Your expertise will support patient recovery, comfort, and well-being through comprehensive assessments, medication administration, and personalized care plans. This position offers an exciting opportunity to make a meaningful difference in the lives of diverse patient populations while working in a fast-paced, collaborative environment. Duties Perform thorough patient assessments, including vital signs, physical examinations, and health histories to determine individual needs. Administer medications and injections accurately following prescribed protocols, including IV infusions and other specialized treatments. Manage catheterization procedures, tube feeding, wound suturing, and sterile techniques to ensure optimal patient outcomes. Assist with diagnostic evaluations such as specimen collection, processing, and medical imaging coordination. Provide specialized care for patients with complex conditions like dementia, Alzheimer's disease, or those requiring memory or behavioral health management. Support discharge planning and coordinate with multidisciplinary teams to ensure seamless transition of care across settings. Maintain detailed medical documentation within electronic health record (EHR) systems like Epic or Cerner while adhering to HIPAA regulations for patient privacy. Skills Proven experience in critical care environments such as ICU or Level I Trauma Centers with acute pain management expertise. Strong background working with diverse populations including geriatrics, pediatrics, neonatal ventilator care, and individuals with developmental disabilities or disabilities. Proficiency in EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks for accurate documentation and communication. Skilled in IV insertion, phlebotomy, venipuncture, catheter care and injection administration using aseptic techniques. Knowledge of
ICD-9/ICD-10
coding for accurate medical recordkeeping and billing processes. Experience in emergency medicine settings including urgent care or emergency rooms with triage responsibilities. Familiarity with infection control protocols and hospice or palliative care practices to support end-of-life patients compassionately. Excellent patient assessment skills combined with physiology knowledge to evaluate symptoms effectively and respond promptly.
