Facility Location Care Centers of Nassau 95146 Hendricks Rd Fernandina Beach, FL 32034 Position Summary The Licensed Practical Nurse (LPN) - Behavior Unit is responsible for providing nursing care and coordinating healthcare services for residents assigned to the Behavioral Unit. This role works collaboratively with the Director of Nursing, Behavior Analyst, physicians, psychiatrists, and interdisciplinary team members to ensure residents receive high-quality care in accordance with their Active Treatment Plans while promoting health, safety, and quality of life. Essential Duties and Responsibilities Provide nursing care and monitor residents in accordance with prescribed Active Treatment Plans and physician orders. Ensure compliance with all federal, state, and company regulations, policies, and procedures. Collaborate closely with the Director of Nursing (DON), Unit Director, Behavior Analyst, physicians, psychiatrists, and interdisciplinary team members to address resident needs. Complete nursing assessments, nursing summaries, care plan updates, and required documentation on a daily, monthly, quarterly, and annual basis. Monitor residents' health status and report clinical concerns to the DON, physicians, and other appropriate disciplines. Coordinate medical appointments, document follow-up care, and arrange transportation through the Duty Administrator. Facilitate communication and problem-solving among members of the healthcare team to ensure continuity of care. Coordinate laboratory testing, consult with physicians regarding orders and frequency, and communicate results appropriately. Immediately report any suicidal ideation, threats of self-harm, or other urgent safety concerns to the DON, physician, and psychiatrist. Administer medications and treatments in accordance with physician orders, state regulations, and nursing standards. Receive, transcribe, and enter physician orders into the electronic health record and communicate pharmacy-related needs. Maintain resident medical records, ensure documentation is complete, and perform chart maintenance according to facility schedules. Complete monthly nursing summaries and update care plans for Level 9 residents or as needed. Attend support plan meetings, interdisciplinary meetings, staff meetings, in-services, and required trainings. Consult with psychiatrists regarding behavioral concerns, medication effectiveness, and mental health needs. Review General Event Reports (GERs) for accuracy and ensure follow-up actions are completed and documented. Communicate concerns, challenges, and opportunities for improvement to nursing leadership. Demonstrate proper use of Safety Care procedures and interventions designed to manage aggression, self-injury, and property destruction. Ensure residents' rights are protected and promote optimal quality of life. Report incidents or suspicions of abuse, neglect, or exploitation in accordance with facility policies and the Disabled Persons Abuse Negligence Act. Maintain professional boundaries and adhere to all abuse risk management and supervision requirements. Perform other duties as assigned. QualificationsRequired Qualifications Current and valid Licensed Practical Nurse (LPN) license in the State of Florida. Ability to effectively communicate and document resident care. Knowledge of nursing practices, medication administration, and behavioral health support. Ability to work collaboratively with interdisciplinary teams. Preferred Qualifications High School Diploma or GED. Six months or more of hospital, rehabilitation, long-term care, behavioral health, or developmental disabilities nursing experience preferred. Physical Requirements & Working Conditions Frequent standing, walking, bending, kneeling, twisting, reaching, and lifting throughout the shift. Ability to lift and carry up to 35 pounds independently and 50 pounds or more with assistance. Ability to push and pull up to 35 pounds independently and 50 pounds or more with assistance. Frequent physical activity in a residential healthcare environment. Exposure to noise throughout the workday. Occasional exposure to bloodborne pathogens and other healthcare-related risks.
