Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and medical history documentation using Electronic Health Record (EHR) systems such as Epic or Cerner. Administer medications and injections accurately following prescribed protocols, including IV infusions and medication management for pain control or chronic conditions. Assist with diagnostic evaluations and procedures such as catheterization, specimen collection, wound suturing, and sterile processing techniques. Provide specialized care for patients with complex needs such as dementia, Alzheimer's disease, or those requiring memory care and hospice services. Manage airway support including ventilator care, tube feeding, and respiratory assessments in acute or long-term care settings. Conduct patient triage in urgent or emergency situations utilizing critical thinking skills to determine priority levels for treatment. Document all patient interactions thoroughly while maintaining strict adherence to HIPAA regulations and clinical confidentiality policies. Collaborate with interdisciplinary teams to develop discharge plans, case management strategies, and long-term care plans tailored to individual needs. Support patients with developmental disabilities or behavioral health challenges through compassionate engagement and tailored nursing interventions. Utilize EMR/EHR systems like Athenahealth or eClinicalWorks to ensure accurate medical records management and compliance with healthcare standards. Qualifications Valid state licensure as a Licensed Practical Nurse (LPN) with current certification to practice in your state. Proven experience working in diverse healthcare environments such as hospitals (including ICU or Level I Trauma Centers), nursing homes, skilled nursing facilities, or home health agencies. Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), CPT coding, and documentation standards. Experience with medical procedures including phlebotomy, venipuncture, catheter care, sterile technique, and basic life support (BLS). Familiarity with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks for efficient patient record management. Ability to handle high-pressure situations with professionalism during acute pain management or emergency room scenarios. Excellent communication skills for effective patient assessment and team collaboration across disciplines such as geriatrics, pediatrics (including neonatal ventilator management), or behavioral health settings. Knowledge of long-term care regulations and experience working with geriatric populations is highly preferred.
