Job Description

Overview Job Duties Under the supervision of Medical Director, Provider or Nurse Manager, the LPN utilizes the nursing process to assist in the provision of ambulatory health care services that reflect the ambulatory health care provider's philosophy and standard of ambulatory health and nursing care of assigned patients. Responsibilities Essential Functions
  • Provides and documents nursing care by planning, organizing, and directing the nursing functions of patients in the clinic. Assesses and monitors patients to identify problem, then documents and communicates this information to provider.
  • With guidance from immediate manager, assists with personnel and payroll functions for assigned personnel including, but not limited to participating in the hiring process, training process and continuous coaching and feedback for the staff in relation to their performance.
  • Initiates, implements and documents patient care in accordance with the guidelines of the nursing department policies and procedures, and the Nurse Practice Act. Promotes and maintains a provider environment that is in compliance with federal, state and local agencies.
  • Plans, provides and documents age appropriate care based on the patient intake by identifying patient problems, implementing interventions based on nursing standards of care, setting realistic goals/expectations for the plan of care, and monitoring/documenting the patient's response to all interventions.
  • Administers and documents medications and intravenous solutions according to established policies. Clinical care is implemented per ambulatory health care standards, policies, and procedures.
  • Coordinates, provides and documents patient and family education. Participates as an active care team member in the coordination of patient care.
  • Complies with patient safety goals and programs as defined in policies and procedures, such as the medication administration process, the safe use of restraints, the use of I.V infusion pumps, the use of clinical alarm systems, communication among caregivers, and accurate identification of patients.
  • Serves as a patient advocate by providing compassionate nursing care, which is individualized to meet the special needs of each patient. Ensures quality and safe delivery of ambulatory health care services.
  • Actively participates in quality improvement activities.
  • Participates as an active team member in the coordination of patient care.
